(a) Data Accuracy and Compliance

  • We request that the information provided in the admission form is accurate and complete.
  • Parents and guardians are expected to abide by the rules and regulations set forth by the daycare center.

(b) Child’s Well-being

  • In the event of accidents or illness, the daycare authorities may seek medical care for the child as required.
  • The daycare authorities will not be held responsible for mishaps of accidental nature.

(c) Photography Consent

  • Parents/guardians give consent to the daycare management to take pictures of their child on daycare premises.
  • These pictures may be used in daycare prospectuses, magazines, or promotional materials with prior notification.

(d) Document Authenticity

  • All documents submitted with the admission form are expected to be authentic originals or true copies as indicated in the checklist.

(e) Compliance

  • Parents and guardians are expected to fully comply with the conditions mentioned above.
  • Providing false documents or incorrect information may result in action taken by the daycare authorities.

(f) Non-compliance and Consequences

  • In cases where any of the above conditions are not met fully or partially, or if false documents or incorrect information have been provided, the daycare authorities reserve the right to take necessary action.
  • Such actions may include striking off the child’s name from the daycare rolls, and the child will be considered withdrawn from the daycare.
